Telluride Whole Health is designed to meet the majority of your health care needs. Although no insurance is required, we recommend that all patients have sufficient protection to cover unexpected health care costs such as ER visits, surgeries, and specialty services.

Some Health shares do not cover pre-existing conditions in full and may have caps on coverage so please be sure to read the plan carefully.
The Patient or Telluride Whole Health may terminate the Membership Agreement at any time for any reason or no reason at all, upon providing the non-terminating Party 30-days prior written notice of such. Practice may terminate this Agreement immediately without notice to Patient if Patient fails to pay his or her Membership fees. Upon termination, Patient can utilize, for a fee, the services of Practice’s outside medical records copying service in the transfer of Patient’s medical records to the Patient’s new primary care. Monthly Membership Fees will continue to accrue and be due under this Membership Agreement until Patient complies with the written notice requirements.
If, for any reason, Telluride Whole Health ceases to offer the Covered Services as listed in the membership agreement or fails to comply with the written notice provisions, then Telluride Whole Health will refund any Monthly Membership Fees (MMF) paid in advance by Patient, for the month(s) following the Practices cessation of Covered Services or failure to comply with the aforementioned written notice provisions. The MMF shall not be pro-rated for any month for purposes of calculating refunds. Monthly Membership Fees will continue to accrue until Patient has terminated the Membership Agreement.
